Roster Information

Each roster must contain the team name, team division, coach, team players first and last name, grade, jersey number, emergency contact person and emergency number.

The roster needs to be completed digitally so all athlete names are correctly spelled/typed for

customized t-shirt orders at the tournament.

A roster should be available at the tournament from the coach on the field upon request.

All rosters must be digitally completed and emailed to

[email protected]

Rosters do not need parent signatures!

All roster forms must be received by September 26, 2019.
